This page explains how the ImmForm team and Public Health England (PHE) uses any information you give to us as a registered user of the ImmForm website, and the way we protect your privacy. Protecting the privacy and personal data of the registered users of the ImmForm website is of the utmost importance to us.
1.1 Statement coverage
This privacy statement only covers the ImmForm website at www.ImmForm.dh.gov.uk. It does not cover links within this site to other websites.
1.2 Compact Policy Statement for use with browsers
ImmForm uses a Platform for Privacy Preferences Compact Policy (P3P CP).
The P3P CP statement is used by Internet Explorer (IE) to determine the actions IE will take, depending on what privacy level the user has selected in their browser. The P3P CP for the ImmForm website can be accessed at www.immform.dh.gov.uk/policy.htm.

2 What information does ImmForm collect about me?
When you register to use ImmForm we store personal information about you, such as your name, email address, telephone number, details about the organisations you work for (e.g. Area Team (AT), Clinical commissioning groups (CCG) or GP Practice etc.), the roles you have at that organisation (e.g. Screening and Immunisation Lead / Manager / Coordinator, Vaccine Ordering contact).
This information is used to create a login account for you and to tell us who you are and what you do. This enables us to give you the appropriate access rights to different parts of the ImmForm website. It also enables us to contact you about the ImmForm services you use, which are accessed from the
ImmForm website, or about other relevant immunisation-related information. When you use the ImmForm website, we use a cookie and collect IP addresses in log files. The cookie does NOT collect any private data. The IP addresses and log files are used for general website management information on usage and may be used to resolve technical issues with connectivity. Log files also allow us to record visitors’ use of the ImmForm website to enable us to enhance the future layout of the site. The data we gather in this way does not contain any personal information or information about which other sites you have visited.
If you provide feedback on the ImmForm website we will only use it to develop and improve the site, or to respond to specific requests (e.g. change of address etc).
3 How do we use your information?
Information collected by us is used for internal review, to improve the content of the ImmForm website, to customise the content and/or layout of the ImmForm website, to notify users about updates to ImmForm website, and to contact users for communication purposes and is not shared with other organisations for commercial purposes.
We will use your personal information for a number of purposes including the

We will keep your information confidential and generally we will only use your information within PHE, NHS England, DH and NHS Supply Chain. However, we do provide some limited contact details where this is context-specific to a limited number of other ImmForm website users (e.g. the Flu Coordinators’ names, email addresses and telephone numbers are displayed on the flu vaccine uptake survey for GP Practices within that AT or CCG only, as described in section 3 above). Only a very small number of ImmForm administrators have access to your data. We do not hold any patient specific identifiers on ImmForm; all vaccine update data are anonymised and aggregated at GP practice level, Trust, AT, CCG, Local Authority level (depending on the particular uptake survey).
5 Your data protection choices
We will only contact you for vaccination or immunisation related services. We intend to start automatically registering all new ImmForm website users as subscribers to the Vaccine Update newsletter. You can opt-out of receiving the Vaccine Update newsletter by emailing the email@example.com mailbox at any stage and from being a registered user on the ImmForm website by emailing firstname.lastname@example.org. We intend to invite ImmForm website registered users that are not currently subscribed to Vaccine Update to subscribe.
If you have an ImmForm website account, you can sign into your account in order to update your personal information.
The ImmForm Website has security measures in place to protect the loss and alteration of information under our control.

7 Glossary of terms
Cookies are files containing information about visitors to a web site (e.g. user name and preferences). This information is provided by the user during the first visit to a web server. The server records this information in a text file and stores this file on the visitor's hard drive. When the visitor accesses the sameweb site again the server looks for the cookie and configures itself based on the information provided.
Log files are files created by a web or proxy server which contains all of the access information regarding the activity on that server.
